Civil Registration

Birth, marriage, and death records held by the Local Civil Registrar

The Local Civil Registrar keeps the civil registry for Naic and issues certified copies of records registered in the municipality. For records registered elsewhere, or for PSA security-paper copies, the office can advise you on where to file.

Services and Requirements

Birth Certificate

Local civil registry copy, or assistance requesting a PSA-issued copy.

Requirements

  • Valid government-issued ID of the requesting party
  • Full name of the registered owner and date of birth
  • Name of parents as recorded
  • Authorization letter and ID copy, if requesting on behalf of another person

Marriage Certificate

Copy of a marriage registered in Naic.

Requirements

  • Valid government-issued ID of the requesting party
  • Complete names of both parties
  • Approximate date and place of marriage
  • Authorization letter and ID copy, if requesting on behalf of another person

Death Certificate

Copy of a death registered in Naic.

Requirements

  • Valid government-issued ID of the requesting party
  • Full name of the deceased and date of death
  • Relationship to the deceased
  • Authorization letter and ID copy, if requesting on behalf of the family

Marriage Licence Application

Filed with the Local Civil Registrar before the wedding.

Requirements

  • PSA birth certificate of both applicants
  • Certificate of No Marriage Record (CENOMAR) from the PSA
  • Valid IDs and barangay clearance of both applicants
  • Parental consent (18–20 years old) or parental advice (21–24 years old)
  • Certificate of attendance at the required pre-marriage counselling

How It Works

Step by Step

  1. Prepare the documents listed above, including a valid ID for the requesting party.

  2. Go to the Local Civil Registrar at the Municipal Hall and fill out the request form.

  3. Submit the form for verification of the record in the civil registry.

  4. Pay the assessed fee at the Municipal Treasurer and keep the official receipt.

  5. Claim the certified copy, or follow the release schedule given at the counter.
