Identification cards, benefits, and assistance for residents who qualify
The Municipal Social Welfare and Development Office administers the national social protection programmes at the local level — identification cards that unlock statutory discounts and benefits, and case-assessed assistance for residents facing a crisis. Applications are assessed individually by a social worker.

Services and Requirements
Senior Citizen ID and Booklet
For residents aged 60 and above, under the Expanded Senior Citizens Act.
Requirements
- Proof of age — PSA birth certificate, valid ID, or other acceptable proof
- Proof of residency in Naic, such as a barangay certificate
- Recent 1x1 or 2x2 photographs
- Accomplished application form from the OSCA
Person with Disability (PWD) ID
For residents with a permanent or long-term disability, under the Magna Carta for Persons with Disability.
Requirements
- Medical certificate or clinical abstract from a licensed physician stating the disability
- Proof of residency in Naic
- Recent 1x1 or 2x2 photographs
- Valid ID of the applicant, or of the guardian for a minor applicant
Solo Parent ID
For qualified solo parents under the Solo Parents Welfare Act.
Requirements
- Barangay certificate confirming solo parent status and length of residency
- PSA birth certificate of the child or children
- Supporting document for the circumstance — death certificate, court order, or similar, where applicable
- Recent 1x1 or 2x2 photographs
- Income statement or certificate of indigency, where required for benefits
Assistance to Individuals in Crisis Situation
Case-assessed help for medical, burial, educational, or emergency needs.
Requirements
- Valid government-issued ID of the client
- Barangay certificate of indigency or residency
- Supporting document for the specific need — hospital bill, medical abstract, death certificate, or school assessment
- Any additional document requested during the social worker assessment
How It Works
Step by Step

Get a barangay certificate establishing your residency and, where relevant, your circumstance.

Gather the supporting documents listed for the assistance or ID you are applying for.

Go to the Municipal Social Welfare and Development Office at the Municipal Hall.

Complete the intake form and sit down with a social worker for the assessment interview.

Submit the documents for evaluation and, for IDs, have your photograph and details recorded.

Follow the release or claim schedule given to you by the case officer.
